I, (name of interviewee), understand that (name of interviewer(s)) will interview me in order to collect historical information about Oberlin, Ohio, for use in History 268, "Oberlin History as American History," taught by Professor Carol Lasser. The particular project for which I will be interviewed focuses on (give specific focus of inquiry–subject, framing dates, etc.).

I understand that the interview will be tape recorded. I understand that (the interviewer(s)) may use portions of the tape as recorded or transcribed for purposes of fulfilling class assignments, including the creation and posting of a paper on the subject listed above on the website of Oberlin College and/or the Oberlin Public Schools. I understand that I may be asked to review portions of the tape or transcript for purposes of accuracy.

I , (name of interviewee) donate the recordings, along with the use of literary and intellectual property rights thereto, to (name of interviewer(s)), who will serve as curator(s) of the materials. I understand that the student interviewer(s) may deposit the tape recording at Oberlin College Archive, which will separately arrange with me the conditions of use thereafter for the tape recordings and/or transcripts, and the conditions of their availability for qualified researchers. I understand that the Oberlin College Archive will not permit access to the recordings until after such a use agreement has been signed.

We, the undersigned, have read the above. The interviewer affirms that s/he has explained the nature and purpose of this oral history research. The interviewee affirms that s/he has consented to the interview, and that his/her participation is entirely voluntary.
